 Panel HospitalsHospital services are provided by:There are Tertiary Care Hospitals available with PHS. These facilities are under contract with PHS for services that are not available within our network. To use these facilities and have the service paid at the panel level of benefit requires an approved referral. This approval must be obtained prior to receiving any service. Referrals can be requested by your Panel Provider or by the insured by calling PHS' Utilization Review Department at (814) 946-7793 or 1-800-422-8929. UR must give approval to the referral to have the benefit paid as a panel level benefit.
